From applicant :
– Bali Visas application form.
– Original passport valid for a minimum of 36 months.
– A minimum of 3 months Kitas validity is required upon submission to immigration.
– Personal NPWP (Tax Filing Number).
– PMA (Foreign Investment Company) company document.
– Limited Stay Permit Card (Kitas) proving that the applicant has lived in Indonesia for the last 3 years.
– SKTT (Temporary Residential Registration).

From company (PMA) :
– Company deed of establishment and its amendments.
– Approval from the Ministry of Law and Human Rights (MoLHR) of company.
– Tax Registration Number (NPWP) of company.
– OSS documents (NIB, Ijin Lokasi, Ijin Usaha).
– Copy of shareholders passport or Indonesian ID card (KTP).
– Company letterhead and company stamp.
